Please fill out this form and submit online. Pickup date is December 19, 2020 between the hours of 1 pm and 5 pm. You must bring identification and proof of children applying for in order to receive your toys. Do not bring children on pick up day. You will receive an email notification of acceptance and approval no later than December 1, 2020.
